Please read the following before submitting our form:
- The clinics we use for vouchers are:
- Fix a Friend Spay and Neuter Clinic Winnabow, NC
- Cape Fear Spay and Neuter Castle Hayne, NC
- Adoption First Veterinary Clinic Jacksonville, NC
- Shoreline Veterinary Hospital Jacksonville, NC
- Pet Vet On Main Wallace, NC
- Our vouchers are for personal pets, or TNR ferals only. We cannot help with pets going into a rescue.
- Please add: lucky@wilmingtonanimalcentrix.org to your contacts, and check your junk folder for our emails.
- Please allow up to 14 days to receive a response.
- Please note that vouchers are nontransferable.
iCare Drive File Manager
Please upload a document showing you are currently enrolled in a public assistance program. We need this to meet state requirements. It ensures we can keep providing support for pets and their families.
Please upload proof that you are in a program such as:
- Food and Nutrition Services (Food Stamps)
- Medicaid
- Work First
- Low Income Energy Assistance (LIEAP)
We need this to follow state rules and keep our program running for pets in need.
